. 6 Object and methods of investigation. During 6 336 , years there were examined 336 patients who were sent to ³ endoscopy departments of Vinnytsya hospitals to clarify . the clinical diagnosis. There were 192 male (57%) and 192 (57%), 144 (43%). 144 female patients (43%). 68 patients with chronic 68 () - atrophic gastritis (CAG) with IM were taken as a main . group because IM was mainly associated with this 30 , . disease. The comparison group included 30 people with , , CAG without IM. The average age of patients examined 52,961,13, in dynamics was 52,96 1,13, average duration of disease 㳿, 1 2013 - 2,60,63 . at the time of IM diagnosis - 2,6 0,63 years. Multiple -ﳿ ﳿ biopsies were performed during endoscopy and 0,5% chromoendoscopy with 0.5% solution of methylene blue ( (two biopsies from the body and antrum and one - from an ) area of the angle of the stomach) taking into account the ѳ requirements of the modified Sydney system of chronic gastritis diagnosis and stained areas of gastric mucosa with the following histological study of biopsies. Biopsy . 10 % material was fixed in 10% neutral formalin and after the routine processing some paraffin blocks were produced, , 57 and out of them - sections made 5.7 microns thick. To . define metaplastic changes of gastric mucosa the : following methods were used: routine histological, , . histochemical. Defining H. pylori persistence has been .ylori , conducted with a rapid urease test, cytology - with a , - Pappenheim stain, from the point of view of histology --ó . Romanovsky-Giemsa and toluidine blue stain.

Immunohistochemical studies were performed - on paraffin sections using streptavidin-biotin method (DAKO, , LSAB2 Systems, ("DAKO", Denmark, LSAB2 Systems, HRP.

HRP). Expression of the mucins profile was determined using MUC 1, MUC5AC, MUC2 antibodies MUC1, MUC5AC, MUC2 and MUC6 MUC6 ( Ma695, CLH2, Ccp58 CLH5, (clone Ma695, clone CLH2, Ccp58 and CLH5, Novocastra, ). 400- "Novocastra "Great Britain). In preparations at 400 magnification we determined the rate of intestinal ( CDX2) 5 differentiation (nuclear label CDX2) in 5 randomly (500 ) selected fields of view ( 500 cells) as a percentage share of positively stained nuclei of epithelial cells of ( gastric mucosa in three compartments (I - surface and ; II , III foveolar epithelium; II - neck region, III - base of the , glands, middle and lower third of the glands to the ). (MUC1, basal sections). To evaluate the expression of mucins MUC5AC, MUC2, MUC6) (MUC1, MUC5AC, MUC2, MUC6) in gastric mucosa - in similar areas there was used semi-quantitative color : 0 () intensity rating scale : 0 (none) - no positive reaction in , 1 () 30% the cells, 1 (weak) - up to 30% of the cells reacted , , 2 () 31 positively, 2 ( moderate) - 31-60%, 3 (strong) - 60% or 60%, 3 () 60% [5].

more stained cells [6].

. Results and discussion. The study of mucins in gastric mucosa showed that a certain cell phenotype , corresponds to some certain type of mucin. So MUC . MU 5AC 5AC is found in the epithelium pits, MUC1 - in 볿 , MUC1 , MU 6 - mucous neck cells, MUC 6 - in the pyloric , MU 2 - exocrinocytes, MUC 2 - in goblet cells. The fact that . , MU 5AC MUC 5AC is selectively synthesized by epithelial cells , of pits and necks of the gland cells of normal gastric , mucosa allowed us to draw a conclusion that the , epithelial cells of the necks of the glands may be . classified as those producing mucus. Due to the low MUC 1 specificity of MUC 1 binding of cytoplasmic mucus, , we observed its weak expression in the cytoplasm of mucous neck cells only in 2 patients with CAG, and 2 , due to lack of positive binding in IM case we excluded it from further analysis. MUC 5AC marking was highly . MUC 5 informative in all the investigated cases. Thus, the . expression of MUC 5AC has not been defined in , MUC 5 columnar epitheliocytes areas of complete IM despite strong and moderate expression in areas with (. 1, ). incomplete IM (Fig. 1a, b).

H. pylori In H. pylori group - associated CAG the MUC MUC 5 . ϳ 5AC labeling was moderate and weak. After eradication of H. pylori infection the expression of 㳿, 1 2013

, observation in cases of complete and incomplete IM (. 4). (Fig. 4). Goblet cells in the initial phases of secretion , were wedge-shaped with the apex, directed to the basal parts, with the foundation towards the lumen of the . stomach. During the observation we found no positive labeling of MUC 2 in the group of patients with gastric MUC 2. MUC6 signet-ring cell cancer. MUC 6 expression was observed mainly in the pyloric exocrinocytes of deep , parts of lamina propria, it was weak in the areas of . incomplete IM and it was lacking in complete IM.

140 .

, Thus the more profound was IM spread, the weaker MUC6, was MUC6 expression, indicating a progressive 볺 displacement of pyloric glands with neogenic intestinal (3 epithelium and severe IM (stage 3 of chronic atrophic ). metaplastic gastritis). While studying biopsies from the fundus we have noted the positive labeling of (. 5), exocrinocytes of the oxyntic glands (Fig. 5), that confirms their restructuring and the development of 糿 (). pyloric metaplasia (pylorization). So the findings of immunohistochemical analysis of mucins profile . indicate some mucin specificity in different IM types.

2, As it can be seen from table 2, when the IM was MUC2 , complete, the MUC2 maximum expression was visible in MUC5AC, MUC1 MUC6 goblet cells, thus MUC5AC, MUC1 and MUC6 marking were absent in columnar epitheliocytes with brush border.
